Hospital & Surgical Insurance
for Academic Year 2004-05 |

1. Being sick in Singapore can be very expensive and a financial drain especially if the illness requires further specialist's consultation, hospitalisation or surgery. This is all the more so for you as international students because you are not eligible for hospitalisation and surgery subsidies. | ||||||||||||||||||||||||||||||||||||||||||
2. NTU has arranged a Group Hospital and Surgical Insurance (GHSI) scheme for all international students. It is compulsory for all full-time international students to subscribe to the GHSI scheme unless you produce documentary proof to show that you are holding other similar medical insurance schemes. | ||||||||||||||||||||||||||||||||||||||||||

3. All full-time international students (non Singapore citizens and non-Singapore Permanent Residents) are required to take up the scheme. Students whose candidature are subsequently converted to part-time will not be covered under the scheme. Singapore citizens/Singapore Permanent Residents who wish to subscribe to the scheme may do so by signing up directly with the insurance company.
Insurance Premium for AY2004-05
Please refer to the attached file. The GHSI scheme does not cover outpatient treatment except for those benefits stated in paragraph 1.2 of the file attached above. Full-time graduate students are allowed to visit NTU Medical Centre for outpatient treatment at a minimum nominal charge of $16 (inclusive of 4% GST) per consultation including standard medication. All other investigations and non-standard medication wil be charged separately.
4. Students who have paid the premium for the whole year (i.e. from July 2004 to June 2005) but
can apply for a refund of the premium paid for the additional month(s). 5. Please submit a written request
to the Office of Academic Services (OAS)enclosing a copy of the letter
certifying the date of completion/withdrawal/conversion to part-time candidature.
7. Complete the Healthcare Medical Claim Form (in pdf format, please click here if you need to download the Acrobat Reader) and attach all original receipts. The claims should be submitted within 60 days of the happening of the event. You should notify ISU with brief details of the claim and the reason for the delay if you are unable to submit the claims within 60 days period. Section III of the claim form has to be filled up by the doctor only for claims that are S$500 or more. OAS will submit the claim to the insurance company and inform the students once the outcome of the claim is known.
